RESUMEN
Antecedentes: El cáncer gástrico difuso hereditario (CGDH) representa un pequeño porcentaje de los casos de adenocarcinoma gástrico con componente hereditario, a diferencia de los esporádi-cos, se asocian a mutaciones germinales en el gen CDH1 y presen-tan un mayor riesgo de desarrollar esta enfermedad a temprana edad.
Presentación de caso: Documentamos el caso de una paciente de la 3 década de la vida, con diagnóstico genético positi-vo para CDH1, por endoscopia con cáncer in situ e historial familiar de cáncer a edad temprana, describimos su manejo quirúrgico y seguimiento. El objetivo de este trabajo es presentar un caso poco frecuente de cáncer gástrico y proponer su manejo de acuerdo a lo documentado en la literatura.
Conclusión: De acuerdo con las actualizaciones de las guías, se recomienda a todo paciente con alteración genética realizar gastrectomía total profiláctica y ser manejado por un equipo multi-disciplinario.

ABSTRACT
Background: Hereditary diffuse gastric cancer (HDGC) represents a small percentage of gastric adenocarcinoma cases with a hereditary component, unlike sporadic ones, they are asso-ciated with germline mutations in the CDH1 gene and present a higher risk of developing this disease at an early age.
Case presentation: We document the case of a patient in her 3rd decade of life, with a positive genetic diagnosis for CDH1, by endoscopy with cancer in situ and a family history of cancer at an early age, we describe her surgical management and follow-up. The objective of this work is to present a rare case of gastric can-cer and propose its management according to what is documen-ted in the literature.
Conclusion: According to the updated guidelines, it is recommended that all patients with genetic alterations undergo prophylactic total gastrectomy and be managed by a multidiscipli-nary team.
